Document Pick-up by Family or Friend

If you receive an email that you have a piece of mail at the CISP office but cannot pick it up or do not want to have it delivered, you may fill out a form to have a family member or friend pick it up for you.

Please email a scanned copy of the form to intlcenter@csuohio.edu or have the person picking up the mail bring a physical copy with them. 

NOTE:

• We do not accept photos of the form, only scanned files
• You may fill out the form electronically, but the signature has to be an official electronic signature.
• We DO NOT accept signatures that are typed. It MUST be ink on paper brought in physically or scanned in an email, or an official digital signature.
