Daily Health Assessment Not Cleared Instructions

Your response to the Daily Health Assessment require you not come to campus. You should stay home or remain in your residence hall room and take care of yourself (10 things you can do to manage  your COVID-19 symptoms at home).

First, look for emergency warning signs* for COVID-19. If you are experiencing any of these signs, seek emergency medical care immediately:

  • Trouble breathing
  • Persistent pain or pressure in the chest
  • New confusion
  • Inability to wake or stay awake
  • Bluish lips or face

*This list is not all possible symptoms. Please call your medical provider for any other symptoms that are severe or concerning to you.

Additional steps required prior to being cleared to come to campus

Students:

  1. Students Living Off Campus: Monitor your symptoms; if you think you are experiencing emergency medical symptoms, or you begin to feel worse, contact health care provider. If you have tested positive for COVID, please notify the CSU Health & Wellness Center at 216.687.3649 or healthandwellness@csuohio.edu
  2. Students Living On Campus: Monitor your symptoms; if you think you are experiencing emergency medical symptoms, or you begin to feel worse, contact the CSU Health & Wellness Center at 216.687.3649 or healthandwellness@csuohio.edu.  If you live in Euclid Commons or Fenn Tower, please also contact the Residence Life team immediately by calling (216) 714-3366.
  3. All Students: Please contact your professor(s) to let them know that you are not able to come to class because you failed the daily health assessment. If you are unable to contact your faculty on your own, please contact the CSU Care Team at magnusacts@csuohio.edu for assistance.


Faculty/Staff:

  1. Monitor your symptoms; if you think you are experiencing emergency medical symptoms, or you begin to feel worse, contact your health care provider.
  2. Contact your supervisor/chair and let them know you are not feeling well. 
  3. If you are able to perform your duties, you may work with your supervisor to continue to work remote or identify remote work assignments, until you are symptom free.

Note: Faculty and staff who are unable to return to work for 3 days must contact Sedgewick at 888-436-9530 to initiate a leave of absence claim.  You may qualify for paid sick leave and expanded family and medical leave for specified reasons related to COVID-19.


Visitors/Vendors:

  1. Monitor your symptoms; if you think you are experiencing emergency medical symptoms, or you begin to feel worse, contact your health care provider.
  2. Notify your CSU contact.
  3. Do not come to campus until you are symptom free for 10 days since symptoms first appeared and symptoms have improved.
