Third Party Billing
If your employer or other agency wants to receive the bill for your educational expenses please complete the appropriate form and submit it no later than the first payment due date of the academic semester. Our office MUST be notified of changes to your academic schedule. Failure to submit your documents timely or to notify us of changes to your schedule may result in late fees being assessed. You must also provide us with documentation from your employer that gives us authorization to invoice on your behalf. Please note, this form is not used if your payment is based on final grades.
