Third Party Billing

If your employer or other sponsor wants to receive the bill for your educational expenses please complete the PDF form below and submit it no later than the payment due date of the semester (Monday prior to classes starting). Our office MUST be notified of changes to your academic schedule. Failure to submit your documents timely or to notify us of changes to your schedule may result in late fees being assessed to your student account.
