Insurance Enrollment and Waiver for International Students
All international students (F-1 and J-1visas) are required to carry health insurance while attending Cleveland State University. Your Treasury Service account will be billed for the UnitedHealthcare Student Health Insurance Plan upon registration for classes.
Enroll in Health Insurance via CampusNet
- Log in and then click personal data > click health insurance > click magnifying glass for the correct year > click choose plan.
- Email Studentbelonging@csuohio.edu if you don't see the choose plan option.
Health Insurance Portal and Insurance card
This plan is underwritten by UnitedHealthcare Insurance Company and is based on Policy #2025-444-2. For a full description of coverage, including costs, benefits, exclusions, any reductions or limitations, and the terms under which the coverage may be continued in force, visit UnitedHealthcare Student Resources for CSU students. You can also view a digital version of your insurance card through this site.
CSU Health Insurance Waivers
Waivers from the university healthcare policy are granted only in the following situations:
- You have insurance coverage because you are sponsored by the U.S. Government, your country's government, or the U.N.
- You have insurance coverage because a parent works in the U.S., your spouse works in the U.S. or you work in the U.S. (Employment will be verified)
- You are currently in the U.S. with a minor F-2 dependent and must be insured by another carrier in order to secure appropriate coverage for your dependent. (Dependent will be verified)
- You are a Canadian citizen or resident.
In addition, for F1 visa students, your current policy must meet the following CSU minimum requirements:
- Medical benefits of at least 80% of the first $250,000 of covered expenses
- Repatriation of remains in the amount of at least $10,000
- Expenses for medical evacuation to your home country of at least $15,000
- Deductible of no greater than $500 per calendar year
- Co-payment or coinsurance not to exceed 20% of covered expenses
- Company providing the coverage meets the required rating and includes a U.S. phone number
How to Waive the CSU Health Insurance
- Go to the Waiver Portal to submit your request before the deadline: Feb. 13
- Print a copy of your waiver submission for your records.
- A completed waiver is not a guarantee that your health insurance will be waived.
- Your insurance must meet all the requirements in order to be waived.
- Your health insurance is successfully waived when the charge no longer appears on your CampusNet account.

Required Tuberculosis (TB) Testing
TB testing is required of all international students at CSU. TB is a lung disease that can be fatal and is spread through the air by an infected person. If you get tested in your home country, be sure the test is recent (within the last six months). Acceptable tests include: QuantiFERON (QFT).
Please email your CSU ID # and results to studentbelonging@csuohio.edu (acceptable file formats: PDF, JPEG, or PNG) OR deliver hard copies to the Division of Student Belonging and Success, SC 319.
Our on-campus CSU MetroHealth Clinic offers TB testing. Call 440-592-1900 to make an appointment for your QuantiFERON (QFT) test at the clinic.
