Financial Aid Forms

The FAFSA and Renewal FAFSA are available to be completed online at www.fafsa.ed.gov. You will need your FSA ID to complete the FAFSA online. If you need a paper FAFSA mailed to you, you may request one by calling 800.433.3243.

Download (Adobe Acrobat Reader required, unless noted otherwise), print, complete and sign the appropriate forms listed below and mail to the following address:

Financial Aid Verification Forms

If you have not already filed your 2016-2017 Free Application for Federal Student Aid (FAFSA), please do so immediately.  If you filed your FAFSA, but did not use the IRS Data Retrieval Tool (DRT), please go back out to www.fafsa.ed.gov and if you qualify, use the IRS DRT to transfer your 2015 tax information.  If successful, the action will elimintate the need for you and/or your parents to submit 2015 tax return transcripts to CSU thereby reducing processing time.

Great News!  The 2017-2018 Free Application for Federal Student Aid (FAFSA) is available beginning October 1, 2016.  Online at http://fafsa.ed.gov.

Families will use their 2015 income to complete the FAFSA.  To avoid having to make corrections, use the IRS Data Retrieval Tool (DRT) on the FAFSA to transfer your 2015 income.

***New for 2016-2017:  Complete your PLUS Application entirely online at StudentLoans.gov!  Parent Borrowers or Graduate/Professional Students will be able to request a PLUS Loan and receive an immediate credit decision.  The online application will be available beginning May 1, 2016.
