Please complete the America Reads Tutor Program Evaluation form found here . When completed email the completed form to:

For Site Supervisors:

**Tutors are required to have their Site Supervisor complete the America Reads Tutor Evaluation form. Please fill out the assessment when notified by the student or America Reads Staff, as the student finishes their placement. The tutors Co-op grade depends upon having this form completed in a timely manner. When completed please email it to:

Site Supervisors are also asked to fill out an America Reads Program Evaluation. This feedback provides valuable information to the America Reads Staff that will be utilized to adjust the program if necessary.
