If any employee or student (or visitor) would like to report a safety or health concern, they are free to contact the Office of Environmental Health and Safety by phone directly. In addition to phone calls, please feel free to fill out a Safety & Health Concern Submission Form and submit it to the Office of Environmental Health and Safety either through campus mail or electronically via email.
NOTE: A name and extension/email address is required in order to receive a formal (written response). The Office of Environmental Health and Safety will address all issues in the manner in which they are received, and will make every effort to respond in a timely manner within the resources allotted.
