Steps to Apply
1. Complete and submit your Scholarship Application form and essay by the deadline. Applications without a complete essay will not be considered for scholarship funding. 300 word minimum, carefully read the essay requirements before submitting. We will send all additional forms to you via email after we receive and accept your application form and essay. (email or fax)
2. Submit your Official High School Transcript, High School Diploma, or GED Certificate. (email or fax)
3. Submit a copy of your Driver License/State ID and current medical/health insurance information. (Email, fax, or bring to your background check at CSU after you are accepted into the program)
4. Sign and submit the Authorization to Release Information Form (email or fax)
5. Submit your COMPLETE Immunization Documentation
• Tuberculosis (TB) Test Results – Current negative test results or positive titer showing the antibodies in your blood.
• Hepatitis B – positive titer results or record of 3 booster series
• Varicella (Chicken Pox) – positive titer results or record of immunization
• Tetanus-Diphtheria (TDap) – Booster within past 10 years or positive titer results
• MMR (Measles, Mumps, Rubella) – positive titer or record of immunization
• Influenza Vaccination
• COVID Vaccination
Email, fax, or bring to your background check at CSU after you are accepted into the program (PHOTOS OR SCREENSHOTS OF DOCUMENTS ARE NOT ACCEPTED, PDF or paper only)
6. Schedule your substance testing after you are officially accepted into the program.
Follow instructions on the form- Bring your substance testing form to the testing at St. Vincent’s. Substance testing forms will be provided to you after you are accepted into the program. Results will be sent directly to us from the testing site.
7. Schedule your background check after you are officially accepted into the program.
Provided in the CSU School of Nursing office- Bring your completed background check form and ID. Background check form will be provided for you after you are accepted into the program
