State in outline form specifically what goals/objectives you would like to achieve while on co-op.

Review your objectives with your site supervisor, and have them sign the report. 

Submit the form on your ESC 300/400 coure's blackboard page. 

Undergraduate Co-op/ Internship Placement Form

Once you have accepted an co-op, or internship, please click the link to the left and complete this form. If you will be enrolled in ESC 300/400 during your co-op, make sure to select "Yes" to the question "Are you enrolled/do you plan on enrolling in ESC 300/400 for this work experience?"; to ensure you are providing the full information required for the class. 
Student Co-op Evaluation

Students please fill out this evaluation at the end of your co-op experience by the due date listed on your semester syllabus.

File Co-op Report Instructions Co-op Reports should be submited to the faculty Mentors (couse instructors) for grading. The graded report may be redistrubuted throughout the university or to local industry partners that are interested in joining the Fenn Co-op Program, take care to not include any confidential information, as well as get approval for the report's release from your supervisor before submitting it to your Faculty Mentor.
Employer Forms Description
Employer Evaluation The individule supervising the student assumes an imprtant role in the student's learning experience by providing feedback to the individual on their performance level with specific suggestions for personal and professional growth. We encourage the Site Supervisor share the results of this assessment with the student.
