Supplement to Graduate Admissions Application for class beginning Spring, 2011

Cleveland State University DPT supplemental application procedure:

  1. Complete the DPT Supplemental Application below, submitting on-line
  2. Complete Prerequisite Excel spreadsheet using downloaded copy, and sending it as an email attachment to ptadmissions@csuohio.edu
  3. Send GRE scores to Cleveland State University (Code #1221)
  4. Complete CSU Graduate College’s On-line Application (must do on-line version of application) here. Application fee will be waived. Select box marked "Send check" to avoid the fee. DO NOT SEND A CHECK!

A baccalaureate degree and all prerequisites for the Physical Therapy Program must be completed by the end of the Fall term before the applicant begins the PT Program. However, students may apply to the program prior to finishing all courses as long as they have finished at least five of the thirteen graded prerequisites courses or have been previously advised on this issue from CSU’s Pre-PT Advising. All prerequisite courses that will not be completed at the time of application submission (before completion of Spring Semester classes) must be listed below, outlining a reasonable plan for completion of prerequisite course work.

Before submitting this application, check to ensure that all of the following have been addressed:

Complete applications will be accepted beginning January 12, 2010 through March 15, 2010 for admission in Spring Semester, 2011. Applications will be reviewed April 15, 2010. Applicants will be informed within 2 months of the review dates.

 

PREREQUISITE COURSE FORM

From your transcripts, please complete the prerequisite course form. Complete the form as follows next to each prerequisite course:

  1. List the name of the college or university where the course was completed.
  2. List the course number (i.e. HSC 476) in the second column.
  3. In the third column indicate the year the course was taken.
  4. Input the credits hours under quarters or semesters as they apply.
  5. Finally, list the letter grade received in that course. Keep the following in mind:
  • When multiple courses fulfill a prerequisite course requirement, choose the course with the highest grade. For example: if you have taken three psychology courses and received A, B and A- respectively, use the two courses with the highest grades (those with the A and A- in this example)
  • If you have taken a course multiple times, use the highest grade received. For example: If you took General Chemistry I and received a D+ and retook the course and received a B+, use the B+ for your calculations.

NOTE: Not all lines in the prerequisite course form may apply to your situation.  Some laboratories are combined with the lecture portion of the courses at some institutions. Please `N/A' out any lines that do not apply to you.

All prerequisite courses must be completed with a grade of a `C' or better to qualify. The final Prerequisite GPA(*) must be a 3.00 or greater on a 4.00 scale. All GPAs will be rounded to the nearest hundredth place with the following rounding rules applied. GPAs ending in 0.004 or less will be rounded down and ending in a 0.005 or greater will be rounded up to the next highest hundredth value.

The overall GPA is calculated using all work completed for the baccalaureate degree as well as any coursework completed after the degree is awarded. If more than one baccalaureate degree is earned, complete the calculations form using the degree with the highest grade point average.
