Application Checklist

We will be accepting applications July 8, 2013 - February 17, 2014 for the class starting in January of 2015 through new Physical Therapy Centralized Application Service (PTCAS) Click here to link directly to PTCAS.

A CSU Graduate Application is required. Applicants may apply online at: http://www.csuohio.edu/gradcollege/admissions/degree.html. Please apply for the SPRING semester of the year that you would ENTER the DPT Program. Please know that it MUST be submitted prior to the DPT Admission deadline, Feb 15 annually (If Feb 15 is on a weekend, the application deadline will be extended to 11:59pm on the immediate next Monday). If this application is not received by the deadline, you will be ineligible. No payment is required (on online application form, check box "Will send check" but DO NOT send a check). Did you complete the CSU Graduate Admissions online application?

The General GRE exam that includes verbal, quantitative and analytical writing sections is required. You must have taken the General GRE exam by Feb 15 annually (If Feb 15 is on a weekend, the GRE deadline will be extended to 11:59pm on the immediate next Monday). If you retake the GRE before this deadline, we will accept the highest score earned in each category. CSU's Designated Institution Code Number is 7740. Have you taken the GRE within the last 5 years and did you have ETS send all official GRE scores to PTCAS using CSU's DPT Program PTCAS code number?

All completed coursework and official grades, including those from Fall 2013, are required to be submitted as part of your application. By Feb 15 annually you must complete the following actions: 1.) Enter in all of your courses and grades, including those from Fall 2013, to PTCAS and submit completed application & pay fees 2.) Hit the ‘E-Submit Academic Update’ button on PTCAS if you are updating your application with newly entered grades 3.) Have official transcripts of all grades, including those from Fall 2013, sent and postmarked on or before Feb 15 to PTCAS (PTCAS, PO Box 9112, Watertown, MA 02471) 4.) Have a second set of official transcripts of all grades, including those from Fall 2013, sent and postmarked on or before Feb 15 to CSU Grad Admissions (Office of Graduate Admissions, Cleveland State University, 2121 Euclid Ave., Cleveland, OH 44115). If ALL official courses and transcripts for Fall 2013 grades are not received by PTCAS and by CSU’s Office of Graduate Admissions by the Feb 15 annual deadline, you will be ineligible for CSU DPT admission. Have you completed each of these steps by the Feb 15 annual deadline?

CSU DPT Admissions decisions will be mailed to the permanent address you provided in your PTCAS application on or before May 1 annually. CSU DPT Admissions often uses applicant's e-mail addresses in communication.

Please see the Pre-Requisite Requirements page for specific info on what pre-req courses must be completed with a B or better and the rule stating that an applicant may earn no more than 2 c’s in pre-reqs.
