I  Graduate College Application Process
(submitted to PH 227 – Office of Graduate Admissions – 216-687-5599 –  call to confirm that all your documents have been received)

1. Application for Graduate Admission form (can be accessed on-line),OR, if an international student, submit Application for Graduate Admission form to the Center for International Services and Programs
   
2. $55 application fee (for both DPT and Graduate Admissions application processing)
   
3. 2 reference forms or letters of reference (forms available on-line)
   
4. Official transcripts from all colleges attended
   
5. GRE test scores submitted by the testing agency directly to CSU
