An application fee of $25 must be paid at the time your application is submitted. This fee does not include the cap and gown fee for commencement. The application fee must be paid at the Bursar's Office, MC 115. Bring the receipt and your completed application to Campus 411. Applications without a receipt and an advisor's signature under College Comments will not be accepted.

If you are mailing your application, enclose a check or money order for $25 payable to Cleveland State University. Mail the check and your application to Cleveland State University, Graduation Office, 2121 Euclid Avenue KB1409, Cleveland, Ohio 44115-2214.

It takes ten to twelve weeks to complete initial processing of the application. When completed, you will receive a copy of the application with comments and adjustments noted. Please check it carefully for approval and/or adjustments. If you do not receive anything back after the ten to twelve week period, contact the Office of the University Registrar.

Undergraduate students can pick up application forms from their college advising offices. Students should contact their College Advising office for assistance in completing the graduation application. Completed application forms must have the College Advisor's signature under the College Comments section.

Follow the instructions attached to the application form carefully. Complete all sections of the form. Direct any questions concerning the university requirements to the Office of the University Registrar. Direct questions relating to the college requirements to your college Dean and questions relating to major/minor requirements to the respective department office.

To prevent delays in processing, include any copies of petitions or waivers with your application. The college Dean for college requirements or the department Chairperson for major/minor requirements must approve any changes in the courses you listed on your application. Please use the Substitution Approval Form.

You must have degree seeking status to be able to file your application. Graduate students are reminded of the regulation that states that a student must be registered in the Graduate College during the semester in which s/he plans to graduate. This rule applies to a student working on the removal of an Incomplete or Not Reported grade from a previous semester since the date of graduation will be at the end of the semester in which the instructor turns the grade into the Office of the University Registrar.

Graduation Application forms are available in the College of Law. Any questions regarding the status of your application should be referred to the College of Law Records Officer.

Following a final audit of your academic record and verification of the successful completion of all degree requirements, the Office of the University Registrar will mail your diploma to the address on file with the University approximately four weeks following Commencement. You are strongly encouraged to verify the accuracy of your name and current mailing address through CampusNet. Please make any necessary address changes online. Your name will appear on your diploma and in the commencement program exactly as it is on the student database.

Your name and degree being received will appear on your diploma, on your official transcript and in the commencement program exactly as it is on the university's database. Please verify the accuracy of your name and degree on Campus Net and if there is an error, changes can be made by Completing the Change of Student Information form and submitted to Campus 411. The Change of Student Information form is available at the University Registrar's Downloadable Forms page.

Duplicate Diplomas may be requested at a fee of $10 per copy. A downloadable request form with instructions is available on the Downloadable Forms page.
