PAYROLL DEDUCTION

FACULTY/STAFF ANNUAL HANGTAG

The payroll deduction form is now available for full-time Faculty/Staff members to purchase an annual hangtag. The Faculty/Staff Annual Hangtag will be valid from July 1, 2007 through June 30, 2009. Click on the link below fill out the Registration and payroll deduction & authorization forms. Send the original signed forms via campus mail or U.S. mail to:

Once Parking Services receives the form it will be processed and your hangtag will be mailed to your home address. To guarantee receipt of the hangtag to your home address by July 1st, forms must be received in the Parking Office by June 17th.

Payroll Deduction Form

Employees with a 9 month or part time contract can go to CampusNet to purchase a hangtag. Please note that in the event of termination, resignation or leave of absence it is the employee's responsibility to return the hangtag to the Parking Office in order to stop payroll deductions.
