“Students are required to have their supervisor complete the Co-op Evaluation form. Please fill out the assessment when prompted by the student or Career Services Center Co-op Coordinator, as the student finishes the Co-op assignment. The form can be found here. The Co-op Student's grade depends upon having this form completed in a timely manner. You may ask the student to return the form to the Career Services Center, fax it to us at 216.687.9313, or save the document as an attachment and email it to Yolanda Burt, Director of the Career Services Center at email@example.com.
The person who supervises the student on assignment assumes an important
role in the student's learning experience by providing feedback to the
individual on his or her performance level with the specific suggestions
for personal and professional growth. In this way, the Site Supervisor,
the Cooperative Education Coordinator, and the academic program form
an effective co-counsel team in assisting the student to reach his or
her professional objectives. To support this relationship we suggest
the site Supervisor share the results of this assessment with the student.
