The Office of International Admissions and Intensive English Language Program have created a common application form for conditional Admission to streamline the process. In order to be admitted into a degree seeking program you must formally apply to degree programs online at http://www.csuohio.edu/admissions/application.html and with the application below we will be able to process the conditional admission into IELP. We must have both applications on file both a decision can be made.

Please note that all graduate applications for conditional admission will be processed but it is at the discretion of the department to offer conditional admission if you meet the departments minimum requirements for admission.

Application Process:

1. Complete the official online application for Cleveland State University and pay the $30 application fee online with a credit card before hitting the submit button. http://www.csuohio.edu/admissions/application.html.
    
2. Complete the application form on this website.
    
3. Send a non-refundable application fee for IELP of $40 with credit card information, a check or money order (drawn from a US bank in US dollars) and made payable to Cleveland State University.
    
4. Official transcripts of academic work and degree certificates from each college/university attended. PHOTOCOPIES ARE NOT ACCEPTABLE. If transcripts are not in English, please include an official/certified English translation of the original documents.
    
5. Letters of recommendation are required for most programs, but not all. Please check with your specific program requirement, undergraduate applications do not need recommendations letters.
   
6. Official financial statements and supporting documents showing sufficient funds are available for at least the first academic year. PHOTOCOPIES ARE NOT ACCEPTABLE. No Certificate of Eligibility (I-20 or DS2019) will be issued without evidence of sufficient financial support.

2 Easy Ways to Pay Your Application fee

Application fees can be paid two ways once you apply.

1. Call (216) 687-2144 to pay by credit card or Fax the information to (216) 687-5299. Please send: Your name as written on the credit card, the credit card number, the expiration date, and the amount to be paid. We accept 
    
2. Mail us a money order made payable to Cleveland State University in U.S. Dollars drawn on a U.S. Bank.

Statement of Financial Responsibility

All international students must show financial (tuition and living
expenses) support for the length of time that they plan to study
(see Costs on website). Financial documentation must be sent in
order for Cleveland State University to issue an I-20 form. The financial verification and I-20 must be presented at the U.S. Embassy or consulate when applying for an F-1 Visa.

Please have two original financial documents prepared:

• One for Cleveland State University and
• One to present to the U.S. Embassy or Consulate.

All documents must be original, dated and issued within the past year. You must have one or a combination of the following documents.

1. An original bank statement in your name showing the amount of funds available in U.S. dollars.
    
2. An original bank statement in your guarantor's name (sponsor or
   parent) that indicates the amount (in U.S. dollars) available accompanied by a letter of confirmation from the guarantor of his or her intention to provide support. The bank statement MUST indicate a minimum amount held in the account, on bank stationary with a telephone and FAX number for the branch and street address.
    
3. An Affidavit of Support (I-134 form) if your sponsor is a US citizen or Permanent Resident of U.S. The form can be obtained from our office or it can be downloaded at http://www.uscis.gov/files/form/I-134.pdf.  Item #11 must be completed indicating the sponsor's financial intentions.
    
4. A letter from your guarantor's employer accompanied by a letter from the guarantor confirming his or her intention to provide support. The guarantor's salary must be at least twice the amount that he or she plans to provide.
    
5. A letter of support from your government or other sponsoring agency that indicates the amount available and additional provisions, such as health insurance coverage. Please provide a signed copy of the award letter with billing instructions. You should also send a copy to Bursar's office once you arrive in order for the billing to take place.
   
   If your documents indicate an amount in a currency other than U.S. dollars we will convert the amount according to the exchange rate on the day we receive your financial support documents
