First of all, you will need to download Acrobat Reader 8.1

In order to submit your form, you will need to install the following 2 files; pdf995s.exe, ps2pdf995.exe

This is a 2-Step Download

1. Free Converter
   Version 1.1
2. Pdf995 Printer Driver
   Version 8.2s

Once these files are installed:

1. Fill out the form
2. File > Print
3. Select PDF995 as your printer
4. Click OK
5. Save file on your computer where you keep your documents
6. Ignore software window or 'X' to clear
7. Email and send as an attachment to the appropriate person
