College Advising Office (ESSC) - Hepatitis B Shots

All Students admitted into the Early Childhood, Physical Education, Mild/Moderate, or Moderate/Intensive teacher licensure programs must submit to or show proof of a series of three Hepatitis B shots. This series of shots takes 6 months to complete.
Since verification of these shots is required for Practicum (or Student Teaching for Physical Education students), it is important that you start this process immediately upon admission into the program.

Proof of completing the series of Hepatitis B shots is required prior to the start of your practicum (or at the time you apply for student teaching for Physical Education students). This proof must be turned in to the Field Services Office in Julka Hall 187 prior to reporting for your Practicum (or student teaching for Physical Education students). If all shots are not completed prior to the start of your practicum, evidence of completing at least shots 1 and 2 should be turned into the Field Service Office (JH 187). Proof of completion of all three shots, however, will be required at the time you apply for student teaching. You may go to your own health care provider or you may contact the Health Services Department in the Center for Innovations in Medical Professions, 2112 Euclid Ave., Room 205 or call at 216-687-3649. If you go to your own doctor, make sure that you get a signed statement from him or her verifying that all three shots were administered along with the date each shot was given. If you have completed the series of Hepatitis B shots previously, verification of this fact by your doctor or health care provider is sufficient and should be turned in to the Field Service Office.

If you choose to use CSU's Health Services Department, these guidelines must be followed:

  1. You must be a registered CSU student.
  2. An appointment must be scheduled.
  3. The fee for EACH shot is $40.00 (for a total of $120.00)
  4. Please allow 6 months for the completion of these shots.

    The timetable for shots is as follows:

    1st Shot
    2nd Shot - 1-2 months after the 1st shot
    3rd Shot - 4-6 months after the 1st shot
  5. Below is a form that should be completed by the Health Services, and the entire form should be turned in to the Field Service Office (JH 187).
