Applicants must complete the participant agreement and send it to the address listed on the form before registration can be confirmed. Participant Agreement form must include your principal's signature.

Upon receipt of the agreement, the Partnering for Success Project Coordinator will register eligible teachers in the Ohio Department of Education's Stars system.
