Monte Ahuja College of Business graduate students who accept scholarship opportunities from the Monte Ahuja College of Business are asked to review the following "scholarship consent information release" regarding release of your confidential information to a scholarship's donor. This confidential information includes: student's name, program of study, academic standing, current GPA, possible graduation date, hometown and scholarship essays. Please note all confidential information is listed on the original application completed by the scholarship applicant - you.
In addition, on rare occasions, the Monte Ahuja College of Business may request to release publication of your name, photography and the scholarship award name and information related to the scholarship requirements. You will be contacted if a photo is requested. You can reject this request in the future. For example, six months after your scholarship has been applied to your CSU student account, if the College makes this request to publish your name and photo, you can simply reject the request.
In order to provide your scholarship donor(s) with the requested information about their scholarship recipients, please select one of the below statements. Your answer has NO impact on your ability to earn a scholarship.

I understand that if any information I have provided is found to be false, it is possible that any scholarship funds that I receive will be rescinded possibly creating a balance owed to Cleveland State University that I will be responsible for paying.
