NOTIFY CAREWORKS MCO Careworks is Cleveland State University’s Managed Care Organization, or MCO. Careworks must be notified of any work-related injury/illness to coordinate and authorize appropriate medical services. Careworks, will work with your doctor to make sure you receive appropriate medical care.If your claim is allowed, Careworks will pay your medical bills. Careworks can be reached at 1-888-627-7586. NOTIFY CSU After receiving medical attention, the employee must complete the “CSU Report of Injury/Illness”. A copy of the report must be returned to Human Resources Benefits unit, AC 113, and Paul Novak, Director of Environmental Services, Physical Plant. The Benefit Application Process The application process includes completion of the Bureau of Workers Compensation “First Report of Injury”, or “FROI”. Typically, your attending physician completes the FROI. CSU employees must also complete the CSU “Report of Injury/Illness” form. CSU will interview your supervisor and any witnesses to the accident/incident, and review medical facts as necessary, before certifying or denying the claim for benefits. Timely completion of the “CSU Report of Injury/Illness” enables us to respond to your claim and provide benefits to eligible applicants. Payment of Medical Bills – If your claim for workers compensation benefits is certified, your medical services will be coordinated by Careworks, CSU’s Managed Care Organization (MCO) for workers’ compensation. Careworks must review and approve all medical services. If the claim is allowed, Careworks MCO will work with your doctor to make sure you receive appropriate medical care and will pay medical and prescription drug bills for authorized treatment. Payment of Lost-Time Wage Compensation - Compensation for lost-time wages is available to you if you are off work for 8 days or longer due to an allowed injury or occupational disease. If you miss 14 consecutive days or more due to an allowed injury or illness, you will be compensated for the entire period of time you are disabled as a result of the allowed injury/occupational disease. The first 7 days of disability are not compensable unless you miss 14 consecutive days. Compensation for lost wages can be paid only after medical evidence is submitted which supports your disability and BWC issues an order approving payment. You may not receive payment for lost time from the BWC if you receive sick pay from CSU. If you have additional questions, please contact the Department of Human Resources at (216) 687-3636, or email the Benefits unit at benefits@csuohio.edu. |
