You have two vision plan options from which to choose:  the “Basic” plan and the “Opt-Up” plan. Both plans are sponsored by Vision Service Plan (VSP), a Preferred Provider Organization. VSP members receive discounted fees for Laser Vision Correction Surgery.  To learn more about these new procedure call VSP or visit their web site:

If you use a VSP provider there are no claim forms to file.  Simply identify yourself as a VSP member when you make an appointment with a VSP service provider.
