Dental

CSU offers a competitive dental plan. The plan includes a Preferred Provider Organization (PPO) and is administered by Medical Mutual (claims payer). When you use preferred network providers you receive 100% coverage for preventive and diagnostic services (i.e. X-rays) and pay no annual deductible.

When you use non-network providers, the plan pays benefits after you pay an annual deductible of $50 per person. You are also responsible for paying any charges that exceed the Usual and Customary Amount (UCR) — or the “going rate.” Preventive care services are covered by network and non-network providers and are not subject to the deductible.

If you are already using a preferred network provider, be sure to show your dental ID card to the office staff so that they know you are eligible for preferred provider discounts — the savings can be significant. To see if your dentist participates in the PPO, for claims questions, to verify coverage, or check the status of a claim, visit http://www.mmoh.com/ or call Medical Mutual at 1-866-336-8251.

Dependent Eligibility for this plan includes legal spouse, same-sex domestic partner (with affidavit), and dependent child(ren) (natural or adopted daughter or son) up to age 23. For other legal health dependents (e.g. step-child, foster child, custodial child, overage disabled dependent child or dependent child of same-sex domestic partner) contact Cleveland State University.
